Clinical analysis on 13 cases of pituitary abscess treated by transsphenoidal surgery
Objective To investigate the treatment of pituitary abscess. Methods Clinical data of 13 cases of pituitary abscess treated by microsurgical transsphenoidal surgery in the second department of neurosurgery of First Affiliated Hospital of Kunming Medical University from March 2008 to April 2015 were collected in this study. Our experience of the surgery and anti-infection treatment were summarized. The concerning literatures were also retrieved and reviewed. Results The collected patients were 6 males and 7 females
at an age of 22 to 59. Their main manifestations were headache
vision loss
hypopituitarism and so on. CT and MRI examination showed that lesions were found on saddle area or on the saddle and saddle areas
Plain scanning showed cystic changes
and was homogeneous or heterogeneous ring enhancement after enhancement. MRI diffusion imaging (DWI) showed high signal unenhanced cystic change
after enhancement was homogeneous or heterogeneous ring enhancement. MRI diffusion imaging (DWI) showed high signal. Pathological observation confirmed that the 13 cases were pituitary abscess. Twelve patients recovered well
and 1 case had postoperative pituitary crisis and died after ventricular system infection. No recurrence was found in the 10 patients who were followed up for 5 months to 6 years
and 2 patients lost to follow-up. Conclusion Pituitary abscess can be treated by transsphenoidal microsurgery and proper anti-infective treatment
