Intranasal endoscopic diagnosis and treatment for spontaneous cerebrospinal fluid rhinorrhea: report of 7 cases
Objective To investigate the diagnosis and treatment method for spontaneous cerebrospinal fluid (CSF) rhinorrhea with intranasal endoscopy. Methods Clinical data of 7 cases with spontaneous CSF rhinorrhea who underwent intranasal endoscopic repair in our hospital from 2000 to 2014 was analyzed retrospectively. They were 2 males and 5 females
at a mean age of 41 (ranging from 19 to 64)
and had suffered from the disease for average 8 months (ranging from 3 to 15 months). The clinical features
body mass index (BMI)
pressure of CSF

surgical treatment and follow-up data were reviewed and analyzed. Results All the patients had been failed with 2 weeks’ conservative treatment
and had to underwent surgical treatment. They were cured by successful surgical repairing under nasal endoscope except 1 patient experienced the operation twice. The intracranial pressure was measured in 1 month after operation by lumbar puncture
the CSF pressure was 64.1~198.5 mmH
2
O
and the average was 135.5 mmH
2
O. The intracranial pressure was measured in 3 months after operation
and the CSF pressure was 105.7~203 mmH
2
O
and the average was 189.8 mmH
2
O.During the follow-up for 6 months to 7 years
no recurrence was found in the cohort. Conclusion Transnasal endoscopy
CT scanning
and MRI are main diagnostic methods to diagnose spontaneous CSF rhinorrhea and make an accurate location of the defect. Transnasal endoscopic management has advantages of less trauma
rapid recovery
fewer complications
and higher successful rate. Accurate location of the defect
appropriate repair method and postoperative management of intracranial pressure are of great importance for successful treatment of CSF leak.
