Anatomy of oblique pedicle screw placement for treating simple compression fracture of lumbar
Objective To discuss the possibility of oblique pedicle screw placement by the anatomic measurement of the lumbar pedicle. Methods The clinical data of lumbar computed tomography of 52 patients from February 2011 to December 2013 in our hospital were accessed. We collected 100 three-dimensional reconstruction data of the lumbar pedicles from 26 of the patients. W
ith these data
we measured the maximum sagittal angle between the screw and the pedicle while the screw broke through the pedicle’s trailing edge at the standard entrance point (with the midpoint of the pedicle as a standard entrance point)
and also measured the sagittal section angle between the screw and the pedicle while the entrance move vertically. Results When using the standard entrance point
the maximum sagittal section angle were: L
1
±18.2°±2.0°
L
2
±15.2°±2.9°
L
3
±14.1°±2.9°
L
4
±15.1°±3.6°
and L
5
±17.4°±3.3°. When moving up the entrance point by L
1
5.2±0.6 mm
L
2
4.2±0.6 mm
L
3
4.2±0.5 mm
L
4
3.9±0.8 mm
and L
5
4.0±0.8 mm
the maximum sagittal section angle were: L
1
33.3°±3.2°
L
2
28.4°±4.8°
L
3
26.6°±4.8°
L
4
28.2°±5.9°
and L
5
31.9°±5.3°
respectively. When moving down the entrance point by L
1
5.2±0.6 mm
L
2
4.2±0.6 mm
L
3
4.2±0.5 mm
L
4
3.9±0.8 mm
and L
5
4.0±0.8 mm
the maximum sagittal section angle were: L
1
-33.3°±3.2°
L
2
-28.4°±4.8°
L
3
-26.6°±4.8°
L
4
-28.2°±5.9°
and L
5
-31.9°±5.3°
respectively. Conclusion Selecting a proper scope of the entrance point with a proper sagittal angle helps place a screw into the uninjured part of the vertebra
which can be used for the treatment of simple compression fracture of lumbar.
