摘要

目的探讨磁共振成像（magnetic resonance imaging，MRI）对线粒体脑肌病（mitochondrial encephalomyopathy，ME）的影像学评价和诊断价值。方法对我院2008年3月及2009年10月收治的2例经临床和穿刺病理证实的线粒体脑肌病的MRI常规扫描和磁共振波谱学(magnetic resonance spectroscopy，MRS )资料进行回顾性分析和总结。结果 1例以共济失调为主要的临床表现，有确切的家族异常病史。肌肉活检阳性，MRI双侧半球对称性的异常信号，MRS双乳酸峰出现；另1例表现为反复的癫痫发作和听力障碍为主，有明显的家族遗传倾向，MRI常规扫描见双侧广泛的脑皮质和白质同时受侵犯的对称性的异常信号灶，伴发脑萎缩等改变，MRS乳酸峰明显升高。结论 MRI检查能比较全面的显示ME神经系统受侵犯的情况，MRS显示乳酸峰的异常对诊断帮助更大。

Abstract

Objective To study the magnetic resonance imaging (MRI) in diagnosis of mitochondrial encephalomyopathy (ME). Methods Clinical data about 2 ME patients

who were admitted from March 2008 to October 2009 and diagnosed by conventional MRI and magnetic resonance spectroscopy (MRS)

were retrospectively analyzed. Results The main clinical manifestations of 1 ME patient with a family history were ataxia and positive muscle biopsy

symmetrical abnormal signals revealed by MRI and double lactate peaks detected by MRS. The main clinical manifestations of the other patient were repeated epilepsy

hearing-impairment

significant family genetic predisposition

and symmetrical abnormal signal foci accompanying brain atrophy revealed by MRI in brain gray and white matter

and increased lactate peak detected by MRS. Conclusion MRI can show ME involving the nervous system and MRS contributes to the diagnosis of ME by showing the abnormal lactate peaks.
