Application of intracranial pressure monitoring in treatment of hypertensive supratentorial hemorrhage patients with small skull window craniotomy
Objective To study the clinical significance of continuous intracranial pressure (ICP) monitoring in treatment of hypertensive supratentorial hemorrhage patients with small skull window craniotomy. Methods Hypertensive supratentorial hemorrhage in 19 patients admitted to first Affiliated Hospital of Chongqing Medical University from January 2009 to December 2010 was dynamically observed by ICP monitoring after small skull window craniotomy. Results Of the 4 patients with severely increased ICP
2 had clinical symptoms and signs and 1 died. The average time of 20% mannitol used was 8±3 days and 11±4 days respectively
while the average dose of 20% mannitol used was 925±125 g and 1 475±175 g respectively in ICP monitoring group and routine treatment group. The time and dose of 20% mannitol used was shorter and lower in ICP monitoring group than in routine treatment group (P0.05). Conclusion Continuous ICP monitoring contributes to the timely and accurate judgment of hypertensive supratentorial hemorrhage in patients undergoing small skull window craniotomy and provides the evidence for its early and correct treatment.
