Surgical treatment for severe thoracolumbar burst fracture without nerve deficit: report of 94 cases
Objective To analyze the clinical effect of surgical treatment for thoracolumbar burst fracture without nerve deficit. Methods A retrospective analysis was made in 94 thoracolumbar burst fracture cases whose never function were Frankel E and who were treated with anterior surgical operations from June 1999 to January 2008. CT images indicated there were more than 50% intraspinal occupying lesion
with bone fragment 0 mm into the vertebral canal The following fixation were used in these cases
such as Kaneda (6 cases)
Z-plate (40 cases)
VentroFix (14 cases)
Antares (28 cases) and other fixation systems (6 cases). There were 71 cases were adopted section of veterbral body and disc which located up and down the vertebral and bone graft
and only upper section of body and disc were resected in 23 cases. Results The never function did not get worse after operation. The average kyphosis degree was 2°
and correct rate was 90%. The average follow-up period was 4.6 years (ranging from 1 to 10 years). A complication due to fixation loose and rupture and the loss of correction did not occurred in one patient. Conclusion The anterior approach is recommended for reconstructing the anterior spinal column in fractures of the thoracolumbar junction
which has many advantages such as the stability reconstruction of spine
