摘要

目的探讨合并腔隙性脑梗死(cerebral lacunar infarction，CLI)的原发性高血压(essential hypertension，EH)患者血压昼夜节律的特征。方法将146例EH患者分为2组：72例合并CLI者为观察组，74例无CLI者为对照组。全部患者作动态血压测定，比较两组间各动态血压值的差异。结果在合并CLI的EH患者中，夜间收缩压和舒张压下降率明显低于无CLI的EH患者，发生血压昼夜节律紊乱的患者明显多于对照组。结论伴有CLI的高血压患者血压昼夜节律的紊乱较无CLI的高血压患者严重，血压昼夜节律的改变与CLI的发生有密切关系。

Abstract

Objective To explore the features of circadian rhythm of the patients in essential hypertension (EH) with cerebral lacunar infarction (CLI). Methods Totally 146 EH patients were divided into CLI group (with CLI

72 patients) and control group (without CLI

74 patients). All the subjects were subjected to ambulatory blood pressure monitoring. Results Systolic blood pressure (SBP) and diastolic blood pressure (DBP) at night decreased more in the EH patients with CLI than that in EH patients without CLI. Abnormal circadian rhythm occurred in more EH patients with CLI than EH patients without CLI. Conclusion Circadian changes of blood pressure was more serious in EH patients with CLI than in EH patients without CLI

suggesting that the occurrence of CLI in EH patients may be associated with the changes of circadian rhythm.
