Diagnosis and treatment of large intracranial artery stenosis and occlusion
Objective To investigate the clinical diagnosis and the application of angioplasty and stenting in the treatment of large intracranial artery stenosis and occlusion. Methods The clinical features
transcranial doppler (TCD) ultrasonography
digital subtraction angiograghy (DSA) and the endovascular stenting of 46 patients with severe symptomatic large intracranial artery stenosis or occlusion were analyzed. Results TCD was sensitive (86.9%) and specific (93.5%) in diagnosing large intracranial artery stenosis. ②Endovascular stent-assisted angioplasty was performed in 46 cases of large intracranial artery stenosis or occlusion
including 24 cases of middle cerebral artery stenosis
17 intracranial internal carotid artery stenosis
1 intracranial internal carotid artery occlusion
2 anterior cerebral artery stenosis and 2 intracranial vertebral artery stenosis. ③44 patients had undergone endovascular stent-assisted angioplasty successfully (95.7%) and their stenosis degree was reduced to 0%-5% after stent-assisted angioplasty. Three cases had severe perioperative complications. No TIA/stroke occurred in 40 cases during a follow-up of 6-12 months. Conclusion Endovascular stenting for symptomatic large intracranial artery stenosis and occlusion is an effective and safe therapeutic method with acceptable complications
but the long-term effect needs to be further investigated.
