Effects of Enalapril or Metoprolol on plasm brain natriuretice peptide in congenital heart disease combined with pulmonary artery hypertension after transcatheter closure
Objective To investigate the changes of plasm brain natriuretice peptide (BNP) in congenital heart disease (CHD) combined with pulmonary artery hypertension (PAH) after transcatheter closure and influence of Enalapril and Metoprolol on them. Methods Totally 119 CHD patients with systolic pulmonary artery pressure (SPAP) higher than 50 mmHg were randomly divided into 3 groups: group A (treated with Enalapril
0.3-0.4 mg·kg

n=44)
group B (treated with Metoprolol
0.8-1.0 mg·kg

n=39) and group C (no drug treatment
n=36). Twenty healthy subjects were used as control. The plasm BNP concentration was assayed respectively by radioimmunoassay before and 4 days
2 months
6 months after transcatheter closure. Both Enalapril and Metoprolol were given at 4
th
day after transcatheter closure. Results Plasm BNP before and 4 days after transcatheter closure in 3 groups and 2 months after transcatheter closure in group C was higher significantly than that in
control group (P0.01). Compared to that before drug treatment
plasm BNP decreased significantly 2 and 6 months after transcatheter closure in 3 groups (P0.01); plasm BNP in groups A and B recovered to control levels 2 months after transcatheter closure
while 6 months in group C. Compared to group C
plasm BNP in groups A and B was significantly decreased 2 months after transcatheter closure (P0.05); no statistical difference of plasm BNP was found between groups A and B at 2 and 6 months after transcatheter closure. Conclusion Plasm BNP increases first
then decreases after transcatheter closure in CHD patients combined PAH. Both Enalapril and Metoprolol accelerate the decrease of plasm BNP.
