Treatment of acetabular defects in total hip replacement
Objective To analyze the curative effect of acetabular defects undergoing total hip replacement (THR) and offer references for intraoperative assessment of host bone stock
bone grafting and acetabular reconstruction. Methods Fifty-eight patients with acetabular defects received THR in our department from January 1998 to January 2004 and were followed up. The efficacy was assessed by Harris hip score and radiographs of the hip joint. Results Among the 58 patients
40 reacted to our follow-up and thus complete follow-up data were got. The average duration of follow-up was 56.00 months (24-70 months). The average age was 56.5 years (23-75 years)
including 22 male and 18 female. The mean preoperative Harris hip score (HHS) was 26.50. The mean postoperative HHS increased to 86.83. The postoperative effect was substantially improved as compared with preoperative one (P0.01). Conclusion This classification can simplify and standardize the evaluation of acetabular defects and guide treatment as well. As a kind of vital bone grafting
the pedicled sartorius muscle iliac bone grafting for reconstruction of acetabular defects can facilitate bone formation. For segmental acetabular defects
morseled bone should be used together with bone grafting. A firm plate and screw system is also essential to achieve good results.
